C\u2019est \u00e0 dire, quels sont les facteurs qui d\u00e9clenchent lipotoxicit\u00e9 cardiaque et comment se<\/div>\n<div>traduisent-ils dans la progression de la maladie? La cardiomyopathie diab\u00e9tique est de plus en plus<\/div>\n<div>reconnue comme une maladie inflammatoire qui est associ\u00e9e \u00e0 une diminution de l&#8217;utilisation du glucose et une augmentation concomitante de l&#8217;absorption d&#8217;acide gras. Ce d\u00e9rangement m\u00e9tabolique r\u00e9sulte m\u00eame en un plus grand rapport de l&#8217;ATP d\u00e9riv\u00e9 d\u2019acides gras contre le glucose que normale.<\/div>\n<div>Malgr\u00e9 cette augmentation d\u2019oxydation accrue des acides gras, des c\u0153urs diab\u00e9tiques accumulent encore des exc\u00e8s de lipides intracellulaires et cela se produit avant le d\u00e9veloppement de dysfonction.<\/div>\n<div>Cela sugg\u00e8re qu&#8217;il existe un degr\u00e9 de \u03b2 -oxydation insuffisante pour r\u00e9pondre \u00e0 l&#8217;augmentation de l&#8217;absorption d&#8217;acide gras qui se produit dans les c\u0153urs diab\u00e9tiques. Ici, nous proposons que les m\u00e9diateurs inflammatoires nuisent \u00e0 la \u03b2-oxydation mitochondriale dans le c\u0153ur diab\u00e9tique qui cause une aggravation d\u2019accumulation des lipides. Avec le temps cette inad\u00e9quation entre l&#8217;absorption et la \u03b2-oxydation dans les c\u0153urs diab\u00e9tiques provoque la progression de la st\u00e9atose cardiaque, qui \u00e0 son tour favorise davantage la lipotoxicit\u00e9 et \u00e9ventuellement la cardiomyopathie diab\u00e9tique.<\/div>\n<div>En effet, des \u00e9tudes cliniques ont montr\u00e9 une corr\u00e9lation entre le degr\u00e9 d&#8217;accumulation des lipides et un dysfonctionnement cardiaque chez les patients diab\u00e9tiques. Nous avons montr\u00e9 que les cytokines r\u00e9duisent \u00e0 la fois l&#8217;activit\u00e9 et l&#8217;expression de r\u00e9gulateurs m\u00e9taboliques cl\u00e9s et des enzymes impliqu\u00e9s dans la \u03b2-oxydation et perp\u00e9tuent ainsi l&#8217;accumulation de lipides. Cela conduit \u00e0 un cercle vicieux de l&#8217;inflammation et lipotoxicit\u00e9 aboutissant \u00e0 la perte de cardiomyocytes, la dysfonction cardiaque et en<\/div>\n<div>fin de compte, l&#8217;insuffisance cardiaque.
